Quilgo Setup guide

1. Install Quilgo Add-on at Google Marketplace (skip if already installed)
2. Go to your Google Forms  and open your form
3. Click the  icon in your Google Form menu, select "Quilgo" and click "Configure"
4. Follow the instructions in the Quilgo configuration sidebar
